Submit everything online
Our appointment request form collects your pet's details and lets you attach bloodwork, radiographs and referral letters directly. It is the fastest route, and nothing gets lost in the post.
What to bring, and what to send
Send ahead if you can
- Recent bloodwork — ideally within the last 30 days
- Radiographs or ultrasound reports
- A referral letter, if your veterinarian wrote one
- Your pet's current medication list, including doses
- Any prior surgical or anesthetic history
Bring on the day
- Your pet, on a lead or in a carrier
- Any medication they are currently taking
- A list of your questions — it is easy to forget them in the room
Fasting instructions
For any procedure requiring anesthesia we will give you specific fasting instructions when we confirm your appointment. As a general rule, withhold food from midnight the night before, and allow water until you leave for the clinic — but follow the instructions we give you, as they vary by age, size and condition.
Other paperwork
Consent forms and estimates are completed with you at the practice, so you can ask about anything before signing. If you would prefer to review them in advance, email us at surgeryforpets@gmail.com and we will send them over.

After-hours emergency
If your pet is in distress outside our hours, call us first — we will direct you to the right care immediately.
When your petneeds surgery, everydecision matters.
Send us the details and any records you already have. We will call you within one business day with a plan and a price — and no obligation to go ahead.
