What we collect
We collect information only when you choose to send it. If you submit the appointment request form, that is:
- Your name, phone number and email address
- Your pet's name, species, breed, age and weight, and anything you tell us about their condition
- Your preferred date and referring veterinarian, if given
- Any files you attach, such as bloodwork or radiographs
We also record a one-way cryptographic hash of your IP address and your browser's user agent string alongside the submission. The hash lets us recognise repeated automated abuse of the form; it cannot be reversed to identify you.
What we do not do
This website sets no advertising cookies, runs no third-party tracking pixels, and does not sell, rent or share your information with anyone for marketing purposes — ever.
Fonts are served from our own domain rather than a third party, so simply reading this site does not report your visit to anyone else.
Why we hold it
To contact you about the appointment you requested, to review your pet's records before the consultation, and to keep an accurate account of what was asked for and when.
Who processes it
Form submissions and uploaded files are stored with Supabase, and notification emails are sent through Resend. Both are established service providers acting on our instructions. Access is restricted to practice staff.
Uploaded medical records are held in private storage. They are not publicly accessible and cannot be reached by guessing a URL.
How long we keep it
Appointment requests that become patients are retained as part of the clinical record for the period required by Florida veterinary record-keeping rules. Requests that do not result in a visit are retained for up to two years and then deleted.
Your choices
You can ask us what we hold about you, ask for it to be corrected, or ask us to delete it, by emailing surgeryforpets@gmail.com or calling (561) 203-0623. If deleting the information would conflict with a clinical record-keeping obligation, we will tell you which part we must keep and why.
Children
This site is intended for adults arranging care for their animals. We do not knowingly collect information from children.
Changes
If this policy changes we will update the date at the top of this page. Material changes affecting how we use information already collected will be notified directly.
